BASIC FUNCTION:

Incumbent is responsible for the provision of culturally relevant health education with an emphasis on clinical prenatal and postnatal care, disease prevention/self-management, and health promotion programs for the Seneca Nation Health System (SNHS).

GENERAL RESPONSIBILITIES:

  1. Plans, organizes, and directs individual and community education programs and events based on the principles of assessment, planning, implementation, and evaluation.
  2. Develops community and individual surveys to help determine up to date needs of the community.
  3. Utilizes needs assessments to advocate for, develop, and conduct, educational presentations to community groups at various worksites and school settings.
  4. Incorporates cultural knowledge into educational programs to encourage patient participation and engagement in services. Provides culturally sensitive and appropriate education.
  5. Prepares and distributes educational and program informational materials.
  6. Serves on committees as needed to promote program services. Collaborates with various agencies and departments both on and off territory to facilitate community events and educational workshops.
  7. Maintains up to date knowledge and skills through the review of relevant literature, current best practices, and through participation in various professional trainings.
  8. Develops and maintains health education libraries to provide resources for staff and community events.
  9. Assists with program audits as needed.
  10. Some evenings and weekends required.
  11. Upholds and promotes the value and integrity of the Seneca Nation Health Systems mission statement. Will promote SNHS in a positive manner.
  12. Provides care through a trauma-informed lens by incorporating key principles of both physical and psychological environment safety, trustworthiness and transparency, collaboration of decision-making during treatment plan developments, empowerment of patients by recognizing and building upon individual strengths, as well as recognizing and respecting the diversity of cultural backgrounds and sensitivity.
  13. Attends all mandatory staff meetings and in-services.
  14. Follows all policies and procedures of the department, Seneca Nation, and SNHS.

KNOWLEDGE, SKILLS, & ABILITIES:

  • Excellent communication, interpersonal and patient service skills to all age groups.
  • Good organization and problem-solving skills.
  • Knowledge of Haudenosaunee or Native American customs and history, or willingness to learn.
  • Ability to work independently required.

QUALIFICATIONS:

  • Bachelors Degree in Health Education or LPN with a minimum 2 years experience providing direct patient care and/or education.
  • Current unrestricted driver license required.
  • CPR/BLS required. Responsible to maintain up-to-date licensure and certification.
  • Experience in a large hospital, academic medical center, outpatient health care setting, or Indian Health Service (IHS)/tribal health, preferred.
