Description: PERSONAL CARE ASSISTANT PRN Location: Norristown, PA

Position Purpose: This role impacts participants lives by providing essential personal care services and health support.
Position Details: Role includes both home care and facility work. Home care assignments would be in the Norristown area with some assignments possibly in Pottstown.
Shifts for coverage would be Mon- Friday 8 am to 430 pm and weekends as needed 8 am to 430 pm.
Must be available to cover at least two weekdays and at least one weekend a month. Schedule is very flexible.
What you will do: - Conduct ongoing participant assessments and monitoring.
- Provide assistance with daily activities, including toileting, grooming, and meals.
- Aid in maintaining physical activity and exercise routines.
- Identify safety hazards and respond to emergencies.
- Assist with recreational activities, meal service, and housekeeping.
- Report any changes in participant's condition to center staff.
- Escort participants to appointments and assist with transportation.
Minimum Qualifications: - High School Diploma or equivalent.
- Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization upon hiring.
- Preferred experience in long-term care or geriatric programs.
- Strong communication and organizational skills.
- Valid driver's license, insurance and use of personal vehicle.
- Ability to work independently and adapt to change.
- Some travel within the region may be required.
